Community Hospice

This service is available for anyone living with any terminal illness in the Darlington and surrounding area.

At St Teresa Hospice we recognise that hospice care is not just about buildings or services – it is a philosophy of care that revolves around you and your family and which can be taken into your own home, hospitals and care homes via our Community Hospice services: Rapid Response, Home Care, Hospice at Home and Volunteer Visitor.

Community Hospice services are important because many people express the wish to be cared for and die at home. However, due to the physical and the emotional demands this can bring, patients can often be admitted to hospital for their last days, which can cause distress to both patients and carers.

We therefore work alongside other community teams to provide care and support in the form of both planned and crisis home visits to meet the needs of the patient and those close to them.

If you require urgent support (response within 1 hour) our Rapid Response Team can be contacted 24 hours a day via the Integrated Single Point of Access (iSPA) on 01325 342111 (select 2 for community services).

This service is available for anyone living with any terminal illness in the Darlington and surrounding area. Referrals can be taken as and when required, and you may be referred several times depending on your fluctuating needs. Community care is appropriate for anyone with a palliative diagnosis.
