Rapid Response

Our Rapid Response Team is there for support when you need it most.

If you have a life-limiting illness and are being cared for at home, we are here to help. The Rapid Response Team offers urgent care – they’re the people to call in a crisis.

The Team offers urgent care at short notice (response within 1 hr) for people living with any terminal illness. Available any time, any day of the week to give hands-on care so people can remain in their homes rather than go into hospital.

Who can benefit from the service?

  • We care for people with a life-limiting illness, such as cancer or respiratory diseases
  • Patients who are 18 years or over and registered with a GP in Darlington
  • Family members or carers of a person with a life-limiting illness.

How can we help you?

Our nurses can help you at any time, particularly during those difficult times at night and weekends. We can help with a range of nursing needs, from overcoming breathing difficulties or pain, to making you comfortable in your bed or chair.

If you are having trouble with any equipment such as a catheter or syringe driver, or just need a friendly face to turn to, we are there for you.

If you require urgent support (response within 1 hour) our Rapid Response Team can be contacted 24 hours a day via the Integrated Single Point of Access (iSPA) on 01325 342111 (select 2 for community services).

Our Rapid Response duty nurse can also be contacted directly on 07720 947430 but please note they may not always be available to answer your call.
